CONSENT FORM FOR FACE PAINTING/TEMPORARY TATTOOS
By signing this form you are giving HB Amazing Faces & Custom Designs your permission for your child to have his/her face painted and/or a temporary tattoo applied and you are indicating your acceptance of the terms and conditions below.
Terms and Conditions:
• Children must be AT LEAST 3 years of age and accompanied by an adult. The adult is responsible for the child and his/her behavior and safety at all times. HB Amazing Faces & Custom Designs is not responsible for supervising children.
• Parent or responsible party Guarantor must signed a consent form for HB Amazing Faces & Custom Designs to face paint each participating child.
• No child will be face painted or tattooed against his/her wishes.
• The products are not suitable for children with sensitive skin or skin conditions.
• DO NOT sign a consent form if your child can not be face painted or tattooed.
• The face paints and tattoos are hypoallergenic/non toxic and comply with FDA/EU cosmetic regulations. HB Amazing Faces & Custom Designs is not responsible or liable at any time for allergic reactions, rashes, hives or other physical symptoms.
• HB Amazing Faces & Custom Designs will accept no responsibility if face paint is transferred or deposited onto clothing, carpets, any flooring, vehicle interiors, or other soft surfaces. To remove from clothing soak in cold water over night with laundry soap before washing as usual.
• The child’s skin must be clean and dry. This is the responsibility of the parent or guardian.
• No eating or drinking during the painting or tattooing.
• The child will not be painted or tattooed if he/she has open cuts, cold sores, chicken pox, measles, mumps,
head lice, chronic runny nose or coughing, conjunctivitis, infectious skin conditions, eczema, or appears unwell.
• Use normal soap and water to remove from skin. Do not use harsh chemicals or make up removal pads.
• HB Amazing Faces & Custom Designs has the right to refuse to paint anyone for ANY reason. HB Amazing Faces & Custom Designs will not paint anything that he/she considers offensive or object able.
Birthday Parties: All remaining balances are due upon arrival before we set up to face paint. All Sales are final. No refunds for face painting or tattoos. We do not accept Checks!! Cash and most major credit cards are accepted. Additional $1.00 charge are added for all credit card transactions.
